Who we are: Home Care Apps Team
Quick answer: Before surgery in Malaysia, ask your insurer or agent ten questions: is the surgery and hospital covered, can you issue a guarantee letter, what will I pay, what pre- and post-hospitalisation and home nursing benefits apply, and what documents and deadline apply. Ask for the answers in writing. Facts checked October 2026.
Key points
- Before you book: ask about cover, hospital network, guarantee letter and room.
- The bill: ask what you pay and what is not payable.
- After surgery: ask about pre- and post-hospitalisation, home nursing and documents.
- Ask for the answers in writing.
Questions 1 to 4: What should I ask my insurer before surgery about cover?
Ask whether the surgery, hospital and doctors are covered, because insurers check policy status, exclusions and waiting periods before they issue a guarantee letter, and AIA lists treatment that can be done as an outpatient among the reasons it may not (AIA). Ask your doctor for the procedure name and planned stay, then ask:
- Is this procedure, for this diagnosis, covered under my policy? The Financial Education Network (FEN) guide says to ask your doctor, insurer or agent (Malay Mail, February 2026).
- Are my hospital and my surgeon on your panel? Great Eastern lists a non-panel hospital or specialist as a decline reason (Great Eastern).
- Can you pre-authorise it and issue a guarantee letter before admission? How early must we ask? One Allianz policy asks for pre-authorisation at least 48 hours before a planned admission (Allianz MediCure). Read what a guarantee letter is.
- What room am I entitled to? A room above your entitlement can lead to "proportionate billing, where you pay a percentage of the total bill" (AmMetLife, March 2026).
Questions 5 to 7: What will the bill look like?
Ask what you will pay, because FEN's guide warns that "having medical insurance does not mean you do not have to pay anything at all" (Malay Mail).
- What are my deductible, co-insurance and limits for this admission? AIA explains co-insurance as "a percentage of the medical expenses that you have to pay".
- Which charges are not payable? AIA gives admission kits, medical report fees, registration fees and vitamins as examples. FEN says not to pay non-claimable items unless they were for procedures you agreed to.
- Will you tell us if the bill is likely to pass a limit, or if the treatment changes? BNM's FAQ says customers will "generally be informed" in that case (C23), and AIA says it may review an approval if the treatment differs. See how to reduce your hospital bill.
Questions 8 and 9: What about pre- and post-hospitalisation and home nursing?
Ask for the number of days and the conditions in writing, because they differ by insurer. Sun Life says "the coverage periods are determined by each individual insurer / Takaful provider" (Sun Life Malaysia, November 2025).
- How many days before admission and after discharge do you cover (pre- and post-hospitalisation), and for what? As one example, the Allianz MediCure wording covers diagnostic tests up to 60 days before admission and follow-up treatment by the same doctor for up to 60 days after discharge.
- Does the policy cover home nursing after discharge, and what must we show? The same Allianz wording asks for nursing within seven days of discharge, prescribed by the attending doctor. Prudential's claim checklist asks for the treating doctor's recommendation letter, the nurse's qualification certificate and a per-day breakdown of the service (Prudential, version 09/2024). Home Care Apps arranges registered nurse home visits.
Question 10: Which documents does the claim need, and by when?
Ask for the document list and the deadline before surgery, because missing papers delay a claim. BNM's FAQ says required documents usually include completed claim forms, medical bills and supporting medical reports, and that processing may be delayed until all are submitted (C19, C20). The FAQ states no claim deadline.
- Which documents will you need, and what is the deadline to give notice and submit? The Allianz MediCure wording asks for written notice within 30 days of a disability that incurs claimable expenses, and proof of hospitalisation within 90 days of discharge. Your policy may differ. See documents needed to claim hospital insurance.
What does the MediAsas pilot FAQ answer?
BNM's pilot FAQ (Version 1, 29 July 2026) answers some of them. MediAsas is voluntary insurance and takaful. As of October 2026 it is in a Klang Valley pilot (BNM), and the final version is scheduled for January 2027, so figures may change.
| Question | What the pilot FAQ says | Still ask |
|---|---|---|
| 2 and 3: network and letter | Cashless at the 10 listed Preferred In-Network hospitals; letter via the provider; if none, pay and claim (B17, C14 to C16) | Is my hospital on today's list? |
| 5: what I pay | Teras: RM500 (age 0 to 59) or RM1,000 (60 and above) per disability; out-of-network adds 20% co-insurance, capped at RM3,000 (B2) | Which plan am I on? |
| 8: pre- and post-hospitalisation | 30 days before admission, 90 days after discharge (Appendix I, items 10 and 11) | Ask for the details |
| 9: home nursing | RM3,000 a year, maximum 90 days per lifetime (item 13). Not stated: how days are counted, or if a doctor's letter is needed | Ask both (more) |
| 10: documents and deadline | Claim forms, bills and reports (C19); no deadline stated | Ask for the deadline |
Who should I ask, and should I ask in writing?
Ask your agent or insurer, and ask for the answers in writing. BNM's FAQ says to contact your agent, insurer or takaful operator, or the hospital's admission counter (BNM FAQ, C11). No source we read requires written answers, but an email gives you a record, and every answer depends on your own policy wording and Product Disclosure Sheet.
- Doctor: is the surgery medically necessary, and is day surgery possible? FEN suggests asking both.
- Insurer or agent: questions 1 to 10.
- Hospital admission counter: how the guarantee letter request is made.
- Employer HR: whether a company medical card also applies.
What to do next
- Email the questions that apply to your insurer or agent.
- Ask your doctor for the procedure name and planned stay.
- Ask the hospital's insurance desk to request a guarantee letter early.
- Keep the written answers with your policy.
- If home nursing is likely, send us an enquiry or try the care assessment.
FAQ
What should I ask my insurer before surgery?
Ask whether the surgery and hospital are covered, whether you can get a guarantee letter, and what you will pay. Then ask about pre- and post-hospitalisation, home nursing, documents and deadlines. The ten questions above give the detail.
Should I ask the insurer in writing?
Yes, where you can. No source we read requires it, but a written reply gives you a record to keep with your policy. Ask for it before the admission date.
Do I need a guarantee letter before surgery?
No, but it is what makes admission cashless. BNM's FAQ says that if a letter is not issued you may still be treated, pay upfront and claim (C16). Ask your insurer early.
Who do I ask, the agent or the insurer?
Either can answer, and BNM's FAQ lists the agent, the insurer or takaful operator and the hospital's admission counter (C11). The insurer's answer is the one that decides the claim, so ask it to confirm in writing.
How we can help
Home Care Apps arranges registered nurse home visits across Peninsular Malaysia, subject to nurse availability in your area. We are not a panel provider of any insurer or takaful operator and do not offer cashless billing, so ask your insurer whether a home visit can be claimed back. Send an enquiry, try the free care assessment or see our services.
