Who we are: Home Care Apps Team
Quick answer: The documents needed to claim hospital insurance after discharge in Malaysia usually are the claim form, original itemised bills and receipts, a discharge summary or doctor's medical report, a copy of your ID and your bank details. Insurers' lists differ, so check yours first. Facts checked October 2026.
Key points
- Core set: claim form, itemised bills, receipts, medical report, ID.
- Insurers' lists differ, so use your own insurer's checklist.
- Medical reports can take two to four weeks; ask early.
- The BNM pilot FAQ gives no claim deadline; check your policy.
What documents do I need to claim hospital insurance after discharge?
You usually need five sets of papers. The BNM MediAsas pilot FAQ (29 July 2026) says required documents "may vary depending on the type of claim, but usually include completed claim forms, medical bills and supporting medical reports" (BNM FAQ C19). Insurer checklists add receipts, ID and bank details, so ask your insurer for its own medical insurance claim form.
| Document | Who gives it | When to ask |
|---|---|---|
| Claim form | Your insurer or agent; the treating doctor fills in the medical part | When you start the claim |
| Original itemised bill and official receipts, including deposit receipts | Hospital billing counter | When you pay the final bill |
| Discharge summary | Ward doctor or nurse | Before you leave the ward |
| Medical report or attending physician's statement | Treating doctor, usually through the hospital's medical report office | As soon as you are discharged |
| ID copy, policy details and bank details | You | When you submit |
Keep the originals together, and make copies before you send anything.
Which documents are required for a health insurance claim at each insurer?
Check your own insurer's list, because the lists differ in detail. Four examples, read on 16 October 2026:
- AIA: copy of NRIC or passport, original official receipt including deposit receipts, original itemised bill, lab and radiology reports, and a report completed by the attending doctor, which the form says the insured person pays for.
- Etiqa Family Takaful: claim form, statement of medical examiner, original itemised bill, original receipts including deposit and refund receipts (marked compulsory), certified true copy of NRIC, and lab results and scans if any.
- Great Eastern General Insurance (the general insurer, not Great Eastern Life): original final bills, original payment receipt, itemised breakdown, an attending physician's statement or discharge summary or medical report, ID and a Direct Credit Facility form.
- Prudential BSN Takaful: include the discharge summary and bills for hospitalisation or surgery, and bills, receipts and the claim form for pre- and post-hospitalisation claims.
The main differences: some name the discharge summary, others only the doctor's statement, and several ask for originals.
How do I get a medical report for my insurance claim?
Ask the hospital's medical report office as soon as you are discharged, and expect roughly two to four weeks. Prince Court Medical Centre says the process takes "about 14 working days from the time all required documents are obtained", with a minimum fee of RM120. Gleneagles Hospital Penang says a standard report is ready within 14 working days, lists a personal medical report at RM161 and a discharge summary as free of charge. Sunway Medical Centre says 2 to 4 weeks, and lists a simple insurance form completed by a specialist from RM90.
These pages are undated and fees can change, so ask your hospital for its own time and fee. Hospitals need the patient's consent, so if you apply for a parent, ask what consent and ID the office wants. See also our hospital discharge checklist.
Which extra documents do special cases need?
Accidents, overseas treatment, balance claims and home nursing can add papers.
- Accident: Great Eastern General's personal accident checklist asks for a police report only if a motor vehicle was involved. The AIA and Etiqa hospitalisation forms do not list one.
- Overseas treatment: AIA asks for a passport copy or flight ticket, and Etiqa for a certified passport copy showing arrival and departure. MediAsas benefits apply in Malaysia only (BNM FAQ C12).
- Balance after another insurer paid: Etiqa and Great Eastern General ask for the other insurer's settlement or final guarantee letter.
- Home nursing after discharge: keep itemised invoices, receipts and visit reports. The BNM FAQ does not say whether a doctor's letter is needed, so ask your insurer first. Home Care Apps says it provides a detailed post-visit clinical report that can be submitted for reimbursement claims, and that cover depends on your plan; your insurer decides what it accepts. Our "from" prices, excluding SST, are on our home page.
Do I still need these papers if the hospital uses a guarantee letter?
Ask your insurer which papers it still needs, but keep them all, because you still check the final bill and may still claim other costs. At a Preferred In-Network hospital under the MediAsas pilot, the insurer pays eligible hospital expenses directly (cashless). You review the final bill and final guarantee letter and pay what the plan does not cover (BNM FAQ C11A).
At an out-of-network hospital, or if no guarantee letter is issued, you pay first and claim with itemised bills and medical reports (C11B, C16). Follow-up bills within 90 days after discharge are a separate benefit (Appendix I item 11), and the FAQ does not say how they are settled, so keep those receipts and ask your insurer. Our guarantee letter guide explains the two routes.
How do I submit the claim, and what if a document is missing?
Work through these steps, and send everything together:
- Before you leave, ask for the discharge summary and the itemised final bill.
- Keep every original receipt, including deposit receipts.
- Ask for the medical report, or ask the treating doctor to complete the medical part of the claim form.
- Fill in your part of the form, and attach your ID copy and bank details.
- Copy or photograph every page before you send it.
- Submit through your insurer's official channel, then track the status there.
The BNM FAQ says processing "may be delayed until all required documents are submitted", and the hospital or insurer will tell you what is missing (C20). The FAQ gives no claim deadline, so read your policy, and see why claims get rejected.
What to do next
- Ask your insurer or agent for its claim form and checklist today.
- Before leaving the ward, ask for the discharge summary and itemised bill.
- Ask the medical report office for the report and its timing.
- Copy every page, keep the originals, and note the date you submit.
- If your doctor advises nursing at home, send us an enquiry or try the care assessment.
FAQ
What papers do I need to claim after my mother leaves hospital?
Usually a claim form, original itemised bills and receipts, a discharge summary or doctor's report, a copy of her ID and bank details. The BNM MediAsas pilot FAQ lists claim forms, medical bills and supporting medical reports (C19). Check your insurer's own list, because lists differ.
What happens if a document is missing?
Processing may be delayed until all required documents are submitted, and the hospital or insurer will tell you what is needed (BNM FAQ C20). Send the missing paper quickly and ask the insurer to confirm it arrived.
Is there a deadline to submit a hospital insurance claim?
The BNM pilot FAQ gives none, and the AIA individual claim form, Etiqa and Great Eastern General documents we read state none. But one AIA corporate-plan claim form (dated July 2018) says 90 days from discharge (see our agent checklist), and Great Eastern Life says most policies include a timeframe. Read your policy and submit as soon as your papers are ready.
Do I need original bills, or is a copy enough?
AIA, Etiqa and Great Eastern General each ask for original bills or receipts. Ask your insurer whether scans or photos are accepted before you send anything.
How do I check the status of my claim?
The BNM FAQ says you can view claim status through the insurer's or takaful operator's official channels once the claim is submitted (C19). Processing time varies with the type of claim and how complete the documents are (C18).
How we can help
Home Care Apps arranges registered nurse home visits across Peninsular Malaysia, subject to nurse availability in your area. Home Care Apps says it provides a detailed post-visit clinical report that can be submitted for reimbursement claims; your insurer decides what it accepts, and we do not guarantee any claim. Send an enquiry or try the free care assessment.
