Who we are: Home Care Apps Team
Quick answer: Use this hospital discharge checklist before taking a family member home: ask the ward doctor or nurse for the discharge summary, medicine list and supply, wound or tube care, follow-up dates and warning signs. Then check the final bill and guarantee letter, collect claim papers and ask about home nursing. Facts checked October 2026.
Key points
- Ask for written instructions, not only spoken ones.
- Collect the discharge summary, medicines and follow-up slip before leaving.
- Check the final bill and final guarantee letter before discharge.
- Ask the ward doctor whether home nursing is advised.
What goes on a hospital discharge checklist in Malaysia?
A good checklist for discharge from hospital covers eight things, and every hospital has its own routine, so ask the ward team how yours works. Hospital Tumpat, a Ministry of Health (MOH) hospital, uses a "Senarai Semak Discaj Pesakit" (patient discharge checklist). It covers the discharge plan told to the patient and next of kin, the discharge note, the prescription slip, the follow-up slip and health education on wounds, diet, exercise and medicines (MOH Hospital Tumpat). Here is a family version:
- Ask the ward doctor or nurse to explain the discharge plan, and to write it down.
- Ask for the discharge summary.
- Ask who will explain the medicines, and how many days' supply you get.
- Ask the nurse to show you any wound, tube or equipment care.
- Get the follow-up dates and any referral letter.
- Ask which warning signs mean you should call, and whom to call.
- Check the final bill and, if cashless, the final guarantee letter.
- Ask for the claim papers, and whether home nursing is advised.
How do I get hospital discharge instructions and the discharge summary?
Ask the ward nurse or doctor before discharge day if you can. The discharge summary is a written summary of the stay, from admission to discharge. MOH Hospital Melaka's discharge procedure (15 January 2018) says a copy of the discharge summary is given to the patient, and that health education is given before the patient leaves (Hospital Melaka).
Private hospitals set their own steps. Gleneagles Hospital Kuala Lumpur's guide lists a discharge blue slip once the bill is ready, payment at the discharge lounge, medicines collected at the nurse counter, and the discharge documents being prepared (GKL guide). If anything is unclear, ask the nurse to explain it before you leave, and ask for the instructions in writing. The US Medicare discharge planning checklist also says to ask for written discharge instructions "that you can read and understand" (CMS, March 2019).
What should I ask about medicines, wounds and tubes?
Ask the nurse to show you, not only tell you, what you will do at home; the US Medicare checklist says the same for tasks like changing a bandage. This is not medical advice: every answer should come from the ward doctor or nurse. For medicines, the MOH Pharmaceutical Services Programme's counselling guideline (third edition, September 2019) includes counselling for patients at discharge, given in the ward or at the inpatient pharmacy (MOH guideline). Questions to ask:
- Which medicines continue, which stop, and what is each one for?
- Do we get a supply or only a prescription, and for how many days?
- How is the wound or tube cared for, and can you show us before we leave?
- When is the next dressing due, and who does it?
- What supplies or equipment will we need at home, and who arranges them?
Hospital Tumpat's checklist includes wound-care teaching and, where needed, removal of a cannula or urinary catheter before discharge.
What follow-up dates and warning signs should I ask about?
Ask for the next appointment date, the clinic and a phone number, all in writing. Hospital Tumpat's checklist includes a follow-up slip and an appointment date, and a referral letter where needed. Ask the ward doctor or nurse which warning signs mean you should phone, visit a clinic or return to hospital, and whom to call after office hours. The US Medicare checklist also tells families to ask what problems to watch for and whom to call. Write the answers on the discharge papers so everyone caring for your family member can see them.
How do I check the final bill and guarantee letter before we leave?
Ask for the itemised bill and read it before you pay. For cashless admission at a Preferred In-Network hospital (a guarantee letter from the insurer), the BNM MediAsas pilot FAQ says to "review your final hospital bill and final Guaranteed Letter carefully before discharge" and to pay any amount the plan does not cover (BNM FAQ C11A). At an out-of-network hospital you may pay first and claim later (C11B). Gleneagles Hospital Penang says bills become due at discharge, and that for insured patients the insurer sends a final guarantee letter to the hospital (Gleneagles Penang).
In May 2025 the Health Ministry said private clinics must give itemised bills on request at no extra cost, citing the 2006 regulations for private hospitals and other private healthcare facilities (The Vibes). Ask your hospital's billing office what it provides. See our guides on the guarantee letter and reducing a hospital bill.
Which papers do I need for the insurance claim?
Collect the discharge summary, itemised bills and receipts, and ask early for a medical report. The BNM FAQ says required documents "usually include completed claim forms, medical bills and supporting medical reports" (C19). For a discharge summary for an insurance claim, Prudential BSN Takaful tells claimants to include the discharge summary and bills for a hospitalisation or surgery claim.
| Paper | Ask whom | Ask when |
|---|---|---|
| Discharge summary | Ward doctor or nurse | Before you leave |
| Itemised bill and original receipts | Billing counter | When you pay |
| Final guarantee letter (if cashless) | Hospital billing or insurance desk | Before discharge |
| Medical report | Treating doctor, through the hospital's medical report office | Request early |
| Pharmacy bills and receipts | Pharmacy | At collection |
A medical report takes time: Prince Court says about 14 working days after all required documents are obtained, and Sunway Medical Centre says 2 to 4 weeks. See the full list of claim documents.
Should I ask about home nursing before we leave?
Yes, ask the ward doctor whether home nursing is advised and for which tasks. MOH Hospital Melaka's procedure says patients are referred for home follow-up care if needed. Under the MediAsas pilot (terms may change before the January 2027 national launch), the post-hospitalisation home nursing benefit is "as charged", up to RM3,000 a year and a maximum of 90 days per lifetime (BNM FAQ, Appendix I item 13). The FAQ does not say whether a doctor's letter is required, so ask your insurer. See how to claim the RM3,000 benefit.
Home Care Apps arranges registered nurse visits. Our published "from" prices on our home page, excluding SST and as of 16 October 2026, are post-discharge monitoring from RM200, wound dressing from RM160 and medication administration from RM127. We provide nurses only, not caregivers or maids (see nurse, caregiver or maid), and we do not supply or refer equipment (see what to buy or rent).

FAQ
What should I ask the hospital before my mum is discharged?
Ask for the discharge plan and discharge summary in writing, the medicine list and supply, wound or tube care shown in person, follow-up dates and warning signs. Then check the final bill and guarantee letter. All questions about her treatment go to the ward doctor or nurse.
What papers do I need from the hospital to claim insurance?
The BNM MediAsas pilot FAQ says documents usually include claim forms, medical bills and supporting medical reports (C19). Ask for the discharge summary, itemised bills, receipts and a medical report, and check your insurer's own list.
Do I need a referral letter for home nursing?
The BNM pilot FAQ does not say, so ask your insurer in writing before the first visit. Also ask the ward doctor whether home nursing is advised, and ask that it be noted in your discharge papers.
Who can help at home after discharge?
For home care after hospital discharge, ask the ward team first: MOH Hospital Melaka's procedure refers patients for home follow-up care if needed. Home Care Apps arranges registered nurse visits only, so for help with bathing or moving around, read our guide to nurses, caregivers and maids.
How we can help
Home Care Apps arranges registered nurse home visits across Peninsular Malaysia, subject to nurse availability in your area. If your doctor advises nursing care at home, we can explain what a visit includes and what it costs, in writing, before you decide. Send an enquiry, see our services or try the free care assessment.
