Before Surgery Checklist for Families in Malaysia: Insurance

Quick answer: Use this pre surgery checklist: ask your insurer in writing what needs approval and what is covered, then gather IDs, the insurance card and a medicine list. Ask the surgeon what care you will need at home, and line it up before the operation. Facts checked October 2026.

Key points

  • Ask the insurer in writing before the date, not on admission day.
  • If no guarantee letter is issued, you may still pay and claim.
  • Bring ID, the insurance card and a list of medicines.
  • Ask the surgeon what care is needed at home.
  • Follow the hospital's fasting rules exactly.

What should we sort out with the insurer before surgery?

Ask your insurer or agent in writing whether the operation is covered, whether it needs approval before admission, and whether your hospital and surgeon are in network. The Financial Education Network's advice, reported by Malay Mail, is to ask your doctor, insurer or agent if your insurance covers the treatment (Malay Mail). Rules differ by policy: Allianz MediCure asks for pre-authorisation at least 48 hours before a planned admission (Allianz MediCure), and Great Eastern lists a non-panel hospital or specialist as a reason a guarantee letter is declined (Great Eastern). Under the MediAsas pilot, the guarantee letter is requested through the Preferred In-Network provider, and if none is issued you may still pay upfront and claim (BNM FAQ C15, C16). The pilot also covers a GP referral and diagnostic tests done within 30 days before admission (C17; Appendix I, item 10). See what a guarantee letter is and questions to ask your insurer before surgery.

Which papers and items should we gather?

Hospitals publish their own lists, so ask the admission counter. Pantai Hospital Kuala Lumpur's guide asks for the admission letter, the patient's NRIC or passport and a valid insurance card, and says all admissions need a deposit, a letter of guarantee or both (Pantai KL). Gleneagles Penang says a letter can replace the deposit (Gleneagles Penang), while Great Eastern says a deposit may still be requested, so ask. Keep everything in one folder.

Item Where from Why
Admission letter Doctor or hospital Asked for at the counter
NRIC or passport You Identifies the patient
Insurance card, policy number Insurer or employer Needed for the letter request
Guarantee letter, if issued Insurer, through the hospital May replace the deposit at some hospitals
List of medicines and allergies Family, doctor Some medicines may need to stop

What should we ask the surgeon and anaesthetist?

The Malaysian Medical Council's consent guideline says a doctor should explain the diagnosis, the treatment options including alternatives, the benefits and risks, the residual effects and the likely result without treatment, and should allow you to seek further explanation (ss. 3.1.2.1 and 3.1.2.6) (MMC). Questions to write down:

  • What is the operation for, and are there alternatives?
  • What are the main risks, and what is the usual recovery?
  • Is it day surgery or an overnight stay?
  • What are the fasting and medicine rules? The Australian and New Zealand College of Anaesthetists says to follow the fasting rules, and that some medicines, such as blood thinners or diabetes medicine, may need to be stopped (ANZCA).
  • What will we need at home: dressings, injections, tubes or equipment?

How do we plan recovery at home before the date?

Ask the home questions now, not on discharge day. Hospital Tumpat, a Ministry of Health hospital, uses a discharge checklist that covers teaching the patient and next of kin about wounds, diet, exercise and medicines (Hospital Tumpat), so expect to learn the tasks. Under the MediAsas pilot (terms may change at the January 2027 launch), post-hospitalisation home nursing is "as charged, up to RM3,000 per year", for a maximum of 90 days per lifetime (BNM FAQ, Appendix I, item 13). Read how to claim the RM3,000.

Home Care Apps arranges registered nurse visits. Our published "from" prices on our home page, excluding SST and as of 23 October 2026, are wound dressing RM160, medication administration RM127 and post-discharge monitoring RM200. We do not supply or refer equipment (see what to buy or rent).

What happens on the day of surgery?

Arrive with the papers in your folder. Pantai Kuala Lumpur's admission counter is open daily from 7.00am to 9.00pm, with admissions after 9.00pm handled at the Emergency Department, and its guide suggests a towel, night attire, non-slip slippers and toiletries (Pantai KL). Follow the fasting rules exactly. If the patient eats or drinks by mistake, tell the nurse or anaesthetist straight away; ANZCA says surgery may be delayed by a few hours so the stomach can empty. Confirm with the admission counter that the guarantee letter, if one was requested, has arrived.

What can family do while waiting?

Ask the ward nurse where to wait and how the team will update you, keep your phone on and keep the papers together. If the plan changes on the day, tell the hospital's insurance desk: AIA says its initial guarantee letter depends on the treatment actually required, and it may review the approval if the treatment differs (AIA). Use the time to write down what the surgeon says afterwards about wound care, medicines and warning signs, because details are easy to forget. This is general advice, not medical advice.

What should we check before we leave hospital?

Ask for the itemised bill and read it before you pay. For cashless admission, the BNM FAQ says to "review your final hospital bill and final Guaranteed Letter carefully before discharge" and to pay any amount the plan does not cover (C11A). Ask for the discharge instructions in writing, collect the discharge summary and the bills, and ask early for a medical report: Prince Court says about 14 working days after all required documents are obtained (Prince Court). Use our hospital discharge checklist and the list of documents needed to claim.

What to do next

  • This week, ask your insurer in writing what needs approval, and ask the hospital to request the guarantee letter.
  • Put IDs, policy details and a medicine list in one folder.
  • Ask the surgeon your recovery-at-home questions and write the answers down.
  • If nursing at home is likely, send us an enquiry before the date.

FAQ

What should my family sort out before surgery so the claim goes smoothly?

Ask your insurer in writing whether the operation is covered and whether it needs approval before admission. Gather ID, the insurance card and a medicine list, and ask the hospital to request the guarantee letter. Check the final bill and final letter before discharge.

Do I need a guarantee letter before surgery?

Not always, and hospitals differ: some ask for a deposit, a letter or both. Under the BNM MediAsas pilot FAQ, if a letter is not issued you may still be treated, pay upfront and claim, under your policy terms.

What questions should I ask the doctor before surgery?

Ask what the operation is for, the alternatives, the risks, the likely recovery and what you need at home. The Malaysian Medical Council guideline says doctors should explain these and allow you to seek further explanation. Write the answers down.

What can family do while waiting during surgery?

Ask the ward nurse where to wait and how the team will update you. Keep your phone on and the papers together. This is general advice, not medical advice.

How we can help

Home Care Apps arranges registered nurse home visits across Peninsular Malaysia, subject to nurse availability in your area. If your surgeon expects dressings, injections or a catheter to be managed at home, we can explain what a visit includes and what it costs, in writing, before the operation. Send an enquiry, see our services or try the free care assessment.
